Hello,

I've got request for additional document 'Receipt of information from client Address history'.
But I'm not sure what it should be a type of document or existing template.

Can somebody give me some advice about that document?

Below is an excerpt from that request:

This is in reference to your application for permanent residence in Canada. In order for us to continue the processing of your application, we require the following documents:





Ø Client Information: Receipt of information from client Address history from 2014-01 to date.



Ø Client Information: Receipt of information from client Address history from 2014-01 to date.



Please note:

All documents must be accompanied by English or French translations. When original documents are requested, notarised documents are not acceptable. When attaching your document(s), please ensure that they are smaller than 5MB and that each document is attached individually.



The above information/documents must be received in our office within 30 days from the date of this letter. Please quote your complete application number on your correspondence and address your correspondence to the address indicated below.

CIC is asking your residence history from January 2014 to till date. Just send an email to CIC with your address history and if convenient attach any mail which has your name and address on it (Phone bill or Credit Card bill). Also do not forget to give the residence history of dependent(s) if any.
