No, you don't have to. Mark it as 'Not Applicable' on the document checklist and you're all set.

No representative - No form - It's that easy.
 
on the check list just bank on forum use representative,right?

As per instruction guide:

Use of a Representative (IMM 5476)
Who may use this form?
Complete this form only if you:

-used the services of a representative to help you prepare or submit your application; or
-are appointing a representative; or
-are cancelling a representative's appointment.

So, 'complete only' means, complete only if you use a representative, if you don't - don't complete the form.

On the checklist don't check the field and write "N/A" or "Not Applicable" next to the checkbox.

It's an application form not an oil painting, but no, it doesn't say anywhere that you must write N/A next to the checklist items that don't apply to you. Just that some people prefer to do this.
 
This form is not required to submit, who don't hire Representative.

I did same as you.
I sent my file 30 september... if something wrong I will get back within 3 weeks .
Now almost completed one month.. We should ok...on our application.
We don't use representative..why do we have submit it, right?

I completed my own application, and I did not appoint a representative. So I did not enclose a Representative form, and I left the checklist item unchecked.

I received an AOR, meaning they considered my application package to be complete.
