Repeat medical exam with another clinic

Hi Everyone

Is it okay if I redo my medical exam with another clinic? My previous clinic required me to do a third X-Ray even with negative sputum results. This would add another three months to my timeline so I thought that if I could redo it with a different clinic, I would at least have a chance to get cleared faster.

Was IRCC the one who asked for another x-ray or the clinic? Did you repeat the x-ray around 3 months after your did your sputum culture? What was the basis for the 3rd x-ray request if you needed to do another x-ray 6 months after your sputum culture?

@canuck78 Yeah, IRCC did as it was recommended by the clinic. I did repeat the X-ray after sputum and this was my 2nd X-ray. The clinic said that since my 2nd X-ray appear to be clearer than my 1st X-ray, I need to do another one (3rd X-ray) after 3 months from my 2nd and since my sputum result is negative and I have provided a personal CT Scan result also showing to signs of tuberculosis, they will not recommend treatment.

Tough to Say whether the clininc is recommending unnecessary tests because you also mention that the 2nd x-ray was clearer implying there was still an issue visible.

The x-ray can remain the same or get better plus a clear sputum culture. Hard to comment without seeing the imaging

Hi same situation. What happend next after you take your 3rd xray? I was ordered also to take my 3rd xray and ct scan even my 2nd xray amd sputum is okay

Is the imaging showing any issues with your lungs other than TB. I have seen a CT being asked to be done but it has been because when TB testing has been completed which has been negative. For many sputum test can be difficult because not everyone can give a good sample so that can be an issue. Normally there are still irregularities on the imaging which is why the CT scan is being ordered. In one case cancer was discovered because irregularities were found on medical after chest x-ray and a CT scan was requested. Have you seen the report from your x-ray and was there any problems found. I would assume there must have been due to sputum testing. Irregularities can be due to previous infections and often mean nothing but knowing if there are any irregularities in the x-rays would be helpful.

Perifissural nodule in the RML 4mm, suggestive of intrapulmonary lymph node.
Parenchymal band in the left lingular segment, suggestive of scarring
# No focal lesion in visible liver or bilateral adrenal glands
No definitely enlarged lymph nodes in mediastinum, lower neck, axilla or upper abdomer
Conclusion: No significant lung lesion in the study.

What happened next ? Can I please know ?
Hello everyone,

This might help if anyone got stuck with medical furtherance.

I just passed my medical after 7 months.
Here is what happened:

I had applied for Spousal PR (outside Canada). I got my medical done on august 12, 2023 and on August 23, 2023, we got email to go back to the panel physician and get additional medical test as asked. PP explained us that abnormalities of about 4.7 grade has been seen in my x ray report, which are suggestive of TB so I have to undergo sputum test(a test for tb) followed by another repeated x ray after 3 months(to check if anything changes or stays stable). If sputum test will come out negative and 2nd x ray shows no changes in the abnormalities seen before only then medical will be approved.

I did my sputum test on August 29, 2023 and was asked to wait for 65 days for culture to grow and it’s results. Also they asked me to see a pulmonologist as recommended by PP who will make my report in the end. He asked me to come to him after 2nd x ray.

In the meantime, we got perspective of another local medical specialist and he did my x ray and told it’s congestion and can be treated with antibiotics. But I was afraid it might change my x ray report and that will make me wait for another 3 months so I didn’t take any medicine.

After 65 days, I called clinic and they confirmed sputum report has been submitted and it’s negative so I went for 2nd x ray and went to pulmonologist as asked.
He told me there is major improvement in the abnormalities in comparison to the previous x ray done on august 13, 2023. So he sent me to the PP again. PP asked me to get ultrasound done in one of their recommended lab. I went and got my ultrasound done and pleural fluid was detected in that due to which they did pleural aspiration( thoracentesis). In this procedure, they use a syringe to remove fluid from the lungs to diagnose the cause of it.
then they sent me home and asked to wait for a few days. After 20 days, when there was no information on my reports, I went to the PP and asked for the status of my medical. He showed me all the results of pleural aspiration. All tests done on the fluid taken( LDH, Protein, Glucose, ADA, Cytology, AFB smear stain, Gram stain, culture and sensitivity) came out negative, hence TB is ruled out. As the report from pulmonologist along with test results was submitted to IRCC, So we have to wait for the decision of embassy now.
A day after we got email from IRCC for additional medical test.
We again went to PP and he told us that IRCC have asked for another repeat x ray on February 13, 2024. We asked PP on what to do, he suggested to consult a chest specialist and get antibiotics treatment done.
We came back and went to top chest specialist (pulmonologist) in our area. He got our ct scan done, and it was found that I have empyema( loculated pleural effusion) which was from previous bacterial infection I had a year ago. Also he got ultrasound done and it showed empyema of about 130-150 ml. So he put me on 3 days iv antibiotics followed by 15 days of oral antibiotics and asked to follow up after a month.
We went again after a month and got ultrasound done, and it showed significant improvement. He put me on 5 days oral antibiotics and asked to let whatever left heal naturally.

I went for my 3rd x ray on February 26, 2024 and submitted the proof of treatment taken along with all ultrasound and ct scans done. I also provided an official document signed and stamped by my personal chest specialist stating no further treatment is required and empyema has been treated. These all reports were submitted to PP as well as pulmonologist and they further submitted the same along with x ray report to Ircc.

After that we got an update in our portal on march 27, 2024, that medical has completed.

Finally, after 8 months we got our medical approved.
So anyone in the same situation, go get check up from your chest specialist and if it is tb or not tb, do take the treatment if suggested any.
Don’t wait for IRCC, consider your health a priority.
