You can email CIC and request reconsideration if you feel there is a good reason (e.g. you were refused because you missed the medical email and failed to do the medical). It's up to CIC whether they re-open your application. Usually they do not.

Sorry to hear that. The same thing happened with "Wahab7", he uploaded all documents but CIC didnt get it. May be you didnt receive any acknowledgement Email.
You have to apply again as a fresh application.
