I am a Canadian Citizen. Due to Family issues, I had to return to India and my health card expired in 2012 August. Now in September 2019, I am returning permanently back to my home in Mississauga.
How to renew, where to renew and what documents required.

Given you have been away for 7 years eligibility may be an issue until you reset residency in Ontario

Reapplying for OHIP
If you’ve been out of Ontario for more than 212 days in any 12‑month period, you may have to reapply for OHIP at a specialized ServiceOntario Centre that provides the full-suite of health card services. You can call ServiceOntario to be sure.

If you are returning permanently you bring proof of residency and proof of when you entered Ontario and you will have to wait the 3 months like everyone else.
