I've never seen this form before, it must be specific for your region. I hope someone can clarify for you! I looked it over and I would ASSUME that would be you... Hrm...

From the Immigration and Refugee Protection Regulations:

11. (1) An application for a permanent resident visa — other than an application for a permanent resident visa made under Part 8 — must be made to the immigration office that serves
(a) the country where the applicant is residing, if the applicant has been lawfully admitted to that country for a period of at least one year; or
(b) the applicant's country of nationality or, if the applicant is stateless, their country of habitual residence other than a country in which they are residing without having been lawfully admitted.

That would also be my reading. Though I find it surprising that if you don't have a spouse they would insist that you certify you also don't have an ex-spouse, without any other option. The instruction guide also gives a very expansive definition of "child", so a person might have children under this definition without realizing it.

You mean you didn't submit IMM5406? It's part of the applicant forms "Additional Family Information". I think this is required for ALL applicants and not region specific.

For Skype logs - I just contacted Skype and they can send your call history on an excel sheet. Then I edited the excel sheet to be only his numbers and his family's numbers.

Never seen it before FS. We don't have any additional family so it really didn't apply to us. Unless it's a new form because I looked it over and it doesn't look familiar at all.

Edit: Just checked my copied one, it's a new form that's why. I do have it where I had to put DH's parents and brother in.

Are you sure? The 5406 we used (and is the latest one from CIC) is dated 04-2008

I checked the checklist for the States, it is listed there as well as the requirement. Check out page A-2, item 4

It's the one where the applicant fills in his/her info about mom, dad, spouse, etc.
 
@Parker24: If you didn't fill that form, the VO will request it later because it is a required form. This is the same form, if I'm not mistaken, that caused a delay for ThunderSoul. It doesn't matter if you don't have any additional family, you still must complete the form and submit it along with all the other immigrant IMM forms.

I'm guessing you did submit it, based on your 'edit'.

I'll double check tomorrow when I'm in the same room as the application (In bedroom now, upstairs, broken foot, need help... I gotta stop talking like a 5 year old haha) But pretty sure it was a different form for the US? I got one form sent back and we had to re-do it but didn't hear anything about another form. Going to check CIC haha.

Edit again: Checked CIC. Yes you need it. Hm. I'm tempted to ask hubby to go get the copied one. It's possible I just forgot what it looked like and I just skimmed through my package. It's been 3 months, almost 4 now, things get blurry in my mind! But will check in the AM before service.

Did you remember signing about having kids or not? That's the form. It's tiny and only one page, so maybe that's why you don't remember it. Not much there to fill really. Even if you didn't send it, I don't think it will be a huge deal. Just fill one and wait for when they ask maybe?

Vaguely remember one asking about his parents and siblings. Nothing about kids, then again we don't HAVE kids so that could be why, I would have just skimmed it. Ack. Worried lol. ???

Nah, don't worry :) you already got your approval, so you're good! When they need it, they will ask.

I specifically remember this because it freaked me out two times: first time when I noticed that we missed that my husband had to sign the section about not having kids, and second time right before sending it, I noticed the first question was asking for his family member names (including mine) in his native script!

Like I said, don't worry. It's not a big deal even if you haven't sent it. You are already approved, and they will ask for missing documents if they need it!

Well, think positive. You figured it out in time. Do you have a case number? If so you can send it to the VO before they ask.

I would freak out, too. But don't worry! It's gonna be fine :)

You mean UCI? Yes. I'm just not sure if it's in LA or not. I got the letter stating it's going there. *grumble* I don't understand CIC sometimes. If this was missing they should have told us *sigh* Should I wait until they ask for it or send it? I just filled it out, with me being the spouse obviously. Hubby is the laid back one, I'm the freaked out one lol.
