"Been refused status ..." would refer to any type of visa or permit to enter a country. The government is clearly trying to find out if the applicant has applied to go to other countries before. Trying to get out of answering them by looking at technicalities is not going to help.
Look at this case: the applicant applied to study in Ireland, withdrew her application, lied about it at the interview, and was found to have misrepresented herself:
