I have 5.5 years of experience under NOC 2172. And, all of my experience is in the same company.

I joined the company as a trainee right after my B-Tech, got confirmed after one year (which changed my designation from trainee). Then I got promotion in 2014 (designation changed). And this year, I was included in a 'center of excellence' team for my technology which changed my designation again. So overall, it is 4 designations. Though the last 2 designations are 2 different names, basically they are at the same level.

So, should I include all these designations, their timelines and specific job duties in the reference letter? Or can I mention my current designation and go with one set of job duties (the responsibilities I have done so far)?

I personally prefer to go with the second option. I am not sure if the employer will agree to give a reference letter with all designations.

Please suggest.

First option is more appropriate, as it will also show salary and duration of each specific role.

include all letters from company from your confirmation letter, it will make your case strong.
