Reason and stage of rejection

I know there's a huge thread of this, but anyone who has been following it, would you be able to tell at what stage most of the rejections happen?

Rejections have happened at each ECAS status including after DM. Most of them happen in In Process (erstwhile In Progress, 1st stage) and Application Received.
