Re-Stamping TRV

Hello, I lost my Indian passport and just got a new one issued to me. I am currently on an open work permit valid till May 2019. How can I get the visa re-stamped on my passport? What are the steps and process because when I applied earlier it was through an agent so I do not want to pay more money and just do it myself this time. I have copy of police report and my new passport in hand.

It's asking for me to upload medical exam. Should I just upload the document the doctor gave me for the one I did when I previously applied?

Have you lived outside Canada in a designated country for more than six month within the past year? make sure you are answering the Questionnaire correctly.

No I have been in Canada since July of 2018. Almost a year now.
