Re-application - IMM 5569

Hello all,
My application for spouse, recently got returned because I haven't met the one of the requirements. I am going to re-submit the application in a few days and have a doubt in the form imm-5569.

under section: 4
question c) made previous claims for refuse protection in canada or at canadian visa board, in any other country or countries or with the UNHCR?

d)been refused refusee status or an immigrant or permanent resident visa or visitor or temporary visa, to canadian or any other country?

For the above two questions do i answer yes or no?

In regards to the returned PR app, the answer is no. It was not a refusal.
