Questions about IMM 5476

I have a few questions regarding the filling out the Use of a representative form. FYI, I am sponsoring my husband, and we plan for me to be his representative since he will be unreachable for a couple of months.

Question #3 asks - Name of office where the application was submitted

Would that be the office they sent it to?? IE: Cairo
or would that be the office I sent it to?? Mississauga

And the declaration: Signature of spouse or common-law partner for sponsorship application

Do I sign here as well?? Even though I will be his representative?? Or is that for 'other' applications???

Also, how do I send it to them, can I send it online??


Actually it's only 2 questions now as I re-read the other question and it says (If known) about the UCI number. And since it isn't known I guess I can leave it blank since we don't know his UCI number.

Sorry for asking such easy questions but I don't want it being sent back because I did it wrong.

I'll give you my opinion...

Question #3, I think you should write Cairo since you are already approved as sponsor and your application is now in Cairo.

The signature: I think it's when an applicant bring his spouse with him. But anyway, it won't hurt if you sign it...

I would send it to the Embassy directly... Nile City Towers, 2005 (A) Corniche El Nile...

Your husband still not have UCI number, I guess you can leave it blank or try to put yours that you received in the SA.

Argh, now I ran out of ink (both black and coloured) in the printer.....

I've heard sometimes they will return docs if there are signatures where there shouldn't be, that's why I was wondering...

I left the UCI blank, and put Cairo in where the app was sent.
