Questions about form 3902E (Part 3, Country Specific Instructions) for UKRAINE

I am a Canadian by birth, born in Canada and am just beginning the process of sponsoring my common-law wife from Ukraine under family class.

1. Is there a national identity document for Ukraine?

2. Do I need to sign the “consent for immigration” form for our children if our children are already all Canadian citizens?

3. For family class immigration (common-law) does my wife still need to provide her Ukrainian labour book and university transcript/diploma?

4. On the CIC website doctors are listed only for Kiev and Lviv which are quite distant from where my wife lives. Are Kiev and Lviv the ONLY places where she can go for a medical in Ukraine?

I am currently sponsoring my wife from Ukraine and we are now in stage 2 waiting for the VO in Kiev to review our file.

1. I believe the National identity document is the inside country passport, as in Ukraine there are 2 passports one for travel and another used as a national ID.

2. Canadian citizens do not need to immigrate to Canada. If traveling with 1 parent only childeren will need a travel consent from the non-traveling parent. (that's standard and has nothing to do with immigration)

3. My wife provided her labor book and education transcripts, that was a country specific requirement.

4. My wife had to travel to Kiev (by plane) to get the Medical examination. When you call to set the appointment you may ask if there are other more convenient locations.
 
I also successfully sponsored my wife from Ukraine, these answers are exactly correct. :D

My wife is from Russia and she needs the same country specific instructions for Ukraine.
I have a question: if she doesn't have a work book, can we just not include a work book or we will have to explain why?

As for the notarized photocopies, it is rather confusing on the country specific instructions about Russia. What exactly should I make notarized and what not? We notarized her birth certificate, her proof of studies. We only made normal photocopies of her passport. Whenever it was neither in English/French, we will have certified notarized translations.

For the proof of the relationship to me, should they be originals or we can scan them, put them in a document and to organize them, then to print them?

Finally, can the forms be photocopied and sent over to me or they need her original signature?

Good to hear you for a very long time..Plz may I ask that when did your file get to Kiev office?Did they send letter of acknowledgement?
