Questions about common-law partner

Dear all,

I am starting now the process for a work permit application (outside of Canada).

I would like to ask you 2 questions:

- I red somewhere that my partner needs to apply by herself. Which form does she need to submit? Does she need to register in the website?
- form imm5409e , part 'Solemn Declaration'. I did not understand how it works. Do you have to do it in a notary at your own country? Do you leave it in blank so it is later filled in Canada?
