Question regarding Federal PR

Hey Guys,

In the PR form, CIC asked Family Name and Given Name from which Family name is mandatory. (Exactly as shown on the passport)

However, In Passport there is no field for Family and whole of my name is under Given name. Surname on passport is empty.

What should i do?

**What is email/call/helpdesk number for CIC for these kind of questions?

Include an LOE stating the discrepancy in your passport and clearly outline your family name and given name. I'd even include a scan of other identification to clearly illustrate your name. Hope this helps.
