Question on the CIT 0002 (10-2017) E New forms of citizenship

Hi ,

i need to urgent help on the below sections on the new citizenship forms .

Q1 : 9a - Dates from the calculator - your eligibility period is xxxx to yyyy
its showing last 5 years date example 10/2012 to 10/2017 , but landed on PR in 2013 , so data i need to fill in 10 a & 11 are from 2012 or 2013?

Q2 : 11. employment and education history - where you physically located ? is it company / institute address?

1) 2012 - entire eligibility period needs to be accounted for, doesn't matter when you landed.
2) work/university/college address

Hi, on question 9 c) should I put the date my work permit expired or the date I became a PR?
For example, I had a work permit valid until June/2015 but in Jan/2015 I became a PR.
