My passport request letter (from Buffalo) contains a section requesting photographs and info on eye and hair color. The items on the list have little lines next to them. I can't tell if these are lines that would be checked if I am required to send the items or if the yjust connote items on a list (like bullet points). I have already sent the pics required with the application and the medical - are they really asking for 3 more??

Has anyone seen this before?

I think requesting more pictures is normal thing...they asked for 4 more from me along with height and eye color and my passport as well.

I can't say what the lines on the form are for though as I didn't get that page from NYC.
 
They also requested additional photos 4 me as well. As far as the 'little lines' i'm unsure of which your reffering to. The littlest lines that mine had were for me to enter height/eye color.
 
Mine has a request for three photographs. The line I am speaking of looks like this:

It looks to me like there would be an X on these lines if I needed to include the information. On the other hand, maybe they are meant to be like bullet points on a list. I guess I will just get 3 more pictures taken at this point and if they don't need them they can throw them out. Thanks.
 
Yes you are going to send in the additional photos. On the little lines that you are speaking of I just put a check mark on them 2 show that I had done it, but theres nothing on the request that states you check them off. Its just a list of what needs to be included along with your passport being sent in.
 
Thanks. I figured I would just to be safe anyway, but good to hear another opinion to confirm. I just can't believe that I have to spend even more $$$$ on photos!
 
LOL ya i know i was like why didnt they just include this total with the application to begin with that way i wouldve already had it ready 2 go, but i guess technially since your pictures are suppose to be recent theres a chance your application couldve taken longer then the 6 month rule they have on photos, but who knows. Don't understand why they request different amounts as well for the ppr.
 
I actually recalled that when I got all of my pics taken originally, they gave me a few extras - so I went back and found them and there were 3, which is exactly what I needed! They are dated June 6,2009 - but all it asks for is "recent" without specifying what that means, so I expect it should be OK - just 2 1/2 months.
 
Yes that sounds ok. I read somewhere in the PR visa application itself and somewhere in the passport application forms they classify recent as less than 6 months old.
