Hello,

If the PA has a child that he does not live with but does pay child support too monthly, do we put him as a dependant child on the application? Does he require a police check and medical exam? He is 4 years old. He doesnt have a passport. Will this pose a problem?

Yes - you must list the child as a dependent (although the child can be a non-accompanying dependent - meaning the child won't come to Canada with your partner). Yes - the child must pass a medical (no PCCs). Yes - I believe a passport is required.
 
Thank you very much! Ive been trying to confirm whether or not the passport for a 4 year old who will not be accompanying PA is required. Can anyone advise?

Check the Guide from which country you belong to. I believe a passport or even a copy of passport of a non-accompanying dependent is NOT required. As scylla said, the child must undergo medical examination but PCC is also NOT required.
