I never used any names so i left it blank but forgot to put in my name at birth... would that affect my application? any answer would be appreciated.

It should be left blank if your name at birth is the same as your current legal name (name listed for question 4a). If not, if you receive the AOR then you should contact IRCC and inform them of the mistake and how to go about correcting it, better be proactive to minimize any delays.
