Query on Additional Family Information.. Native Language

Additional family information form mentions all name to be completed in native language . Since there is no space in form, I plan to add a separate sheet with two columns 'Name in English' and 'Name in Native', sounds right !! Doesnt' it?

Yes... It should be good enough... mention the relationship also for each family member.
