Query about medical health assessment (PAKISTAN)

I am from Karachi, Pakistan and I have been invited to apply for Canadian Permanent Residence visa through Federal Skilled Worker Express Entry Stream.

I have to go through medical health assessment as a requirement, the details of which are mentioned in the following website.

The link mentions following ways as to go through medical examination

1) Wait for instructions after submitting the application

2) Undergo medical exam before submitting the application


could you please tell me which one applicable for me to undergo my medical health examinations being a citizen of Pakistan, Karachi?

also please tell the complete requirements for booking medical health examination and essential documents to be brought along at the examination day.
