query about fees payment form

Dear seniors
I am paying the fees by demand draft.The DD in made from my mother in laws bank account.
so in Name of payer and address of payer should I write the name and address of my mother in law.

Also there is no option for DD details such as dd no drawn bank etc so should I write my name on the back of the Demand draft.

Seniors please guide as I want to courier the forms to CIO by two days.

Doesnt matter from which a/c a DD is made. Main name will be the payee (cio in this case). so you can give u r name & address.
