hi,
i am applying for CSQ from india...i have my dependent child and accompanying wife...
I have completed main form for principle applicant along with my wife and dependednt child of 2.5 years.
Now i wnt to obtain extra point for Spouse and dependent child indicated in principle application.
Do i need to fill another form for spouse i.e there in Quebec immigration site name is " Application for Selection Certificate – Dependent child aged 22 or older or who is married or a de facto spouse" ?

or
can I include my Spouse's CV and kids Birth certificate along with alone principle applicant form ?
Please advice as i dont wnt to pay huge amount to immigration consultnts

Thanks

I have only just starting to gather my documents and filling the forms myself.
This is what I am doing:
I am filling 2 forms - 1 for main applicant and 1 for the dependant de facto spouse.
1. On each form you should mention who is this form for (i..e main applicant or spouse/de facto spouse)
2. and in the declaration where you say you are married to or cohabiting with this person, you should both sign this declaration on each others forms.

So 2 application forms should be sent.

Edit: Just wanted to add to that, on the application form itself, the first page containing the instructions, where it says "Who is requiered to fill out this form" it says that this should be filled by the applicant AND spouse / de facto spouse.

hello malkeet,
how are you, bonne chance en avance,
you should fill two Application for Selection Certificate , one for yourself indicating principal applicant & other indicating accompaying spouse, no need for child under 22.

p.s. send birth certificates of all.
