We do not know all of the exact dates on which we visited each other because we always just simply drove across the boarder. We have also visited more than 5 times and that is all that it allows us to put. What should we put down for dates since we don't know all of the exact dates we visited each other?
 
Take your best guess. On an additional sheet, list the other visits and explain that you can't recall exact dates. It won't be an issue.

There has been close to 100 visits between the 2 of us. It's just difficult to remember that many dates!

I suggest you list the longer visits and the ones you remember dates for. Then explain that there was also approximately X amount of shorter visits, i.e. "approx 3 additonal short visits every month as well", but you cannot recall the exact dates.

1 more question, do I have to include my trips to the USA? Or just her trips to Canada?

This often doesn't work for land crossings. I've entered Canada five times by land (Port Huron and Windsor) since December of 2015 and nothing shows up, nor do my daughter's three crossings.
