I understand from the CIC website that in order to prove relationship to a relative - "An official document naming the applicant as a relative" should be declared.
My question here is, Who should declare and sign the affidavit ? Relative or the applicant?

I don't think an affidavit will get you anywhere. That's not what they mean by official document.
It needs to be both your birth certificates to show same parents, or family booklets if you have those in your country.

My sister and me have the birth certificate but the father's name is a little different on our certificates. For my sister, father's name written is Ram Pravesh Singh and on my certificate its Ram Pravesh

What about passport? Is your father's name the same on both your passports?

Provide letter of explanation describing the difference, and ideally, additional document that shows the same name.
Like his passport for example.

Would a one and same person affidavit by my father work?
Also, he does not have a Passport issued yet. Would Aadhar Card, Pan Card or Driving License work as replacement to Passport as his ID Proof ?

Yes you can add an affidavit from him.
Aadhar and pan cards, I don't know what those are sorry.
Driving license, yeah sure, if it supports the name difference issue.
