Proof of payment

Do i just include the receipt as proof? IMM5401 What form is this? as in the check list its asking me to show this? I paid my fees on line.

Hi, when you pay, it generates the receipt, just print it off and fill it in and attach it to your application , that's it

which part exactly do you fill in? i paid online and printed the receipt and theres nothing to fill in?

There should be space in the PDF you receive after paying online to fll in your name, date of birth, address.
