What type of information should be included in the letter from the bank?

The bank letter needs to have the following:

1. Account Name
2. Opening Date
3. Current Balance
4. Avg Balance for 6 months
5. Outstanding debts or lein against account identified
6. Be printed on bank letter head with their contact details outlined.

Hi All,

If someone could help me with information regarding POF, Me and my wife have FD more than a year old of around 20000$. I do not have sufficient funds in my saving account at the moment. My question is that if the FD certificates would be sufficient as POF in this case or do I still need to provide bank statement.

Hi everyone
Today I went to the bank and they only offered me the 6 months back statement with my name,, account number and current balance,,
Bank contact address is not mentioned
They refuse to give me any letter or a proof that such letter is not available
Also my name in the statement is not in English.
Is bank statement enough as a proof of funds??
And should I translate the bank statement into English??
