Prohibitions under the Citizenship Form? PDF?

Hey Guys,

So at long last I got my notice to appear to be sworn in as a Canadian Citizen, and I've been looking at all the documentation I need to bring with me, but the only one I had a hard time finding was this one: The Prohibitions under the citizenship form. Try as I might, I was not able to get a PDF of that. Or can I just find the questionniare and print it in word form, filled out and stuff?
