I am a Canadian Citizen. My wife (Indian Citizen) lived with me in Halifax from August 1988 to February 1993 as a visitor, and from February 1993 to December 1995 as a Landed Immigrant. Since then she has been living outside of Canada. Now she is interested to return to Canada. Please email me application form and procedures to reactivate her permanent residency status.

Have the two of you been living outside of Canada together since 1995?
 

You don't need to "reactivate" her status. She still has valid status as a PR and because you have been living together, she is still in compliance with the PR Residency Obligation.

She needs to apply for a PRTD and show that she has been residing with her Canadian spouse while outside of Canada.

You need either a valid PR card or a valid PR Travel Document. Unless you fly to the US and enter Canada through a land border using a private vehicle - then you can use your COPR.

SIN is federal. Province is irrelevant.
 
The only thing SIN would tell about you is either where were you born or where you you get it. But after that it is not relevant anymore.

I am told that to obtain Canadian PRTD it might take upto few months.

Does getting Canadian visit visa affect the chances of getting PRTD later.

If my wife comes to Canada on visit visa, can she then apply for PRTD? I would appreciate your feedback.

As your wife is still a PR she cannot apply for a visit visa only a PRTD from outside of Canada
