Procedural Fairness Letter - Unexpectedly! Requesting me for documents I did not receive

Hello,
I am a bit panicking because my PR may be terminated as per Procedural Fairness Letter. What I am mostly surprised that I am considered late on documents I was not asked to update / provided. I'm granted a last opportunity to submit documents I was did not receive nor asked before to update. I'd appreciate if anyone had a similar experience to share it and advise me on what to do as I am very confused.
on 30th Jun 2021, I was requested in two messages for the following:

• Update my wife's information - DONE (and was told X-ray test for my wife will be sent)
• Redo my x-ray x-ray test - DONE

I submitted along her information that she was pregnant and her delivery is due end of year. I was sent another message requesting baby's information once delivered, and, an X-ray examination will be sent:

• My wife delivered and now I have just collected my baby's passport. So I should be submitting this.

Unfortunately, I find my self receiving a message titled "Procedural Fairness" last weekend that I did not receive a notification for receiving. and its telling me that on 30th Jun 2021, I missed the following:

• My wife's X-ray Form (Which I was not sent)
• Kit, Application forms sent to me (which I was never requested to fill or been sent)
