problem in supplementary information form

Hello Everyone,

I received the request of the supplementary information on behalf of getting my study permit.

In this form, there are questions related to my research/work activity in my home country which is Egypt. In the form, they mentioned my country as Iran which is wrong.

(Supervisor in Iran for previous studies)

Shall I complete the form and submit it as it is or do I need to mention in every question related to "Iran" that I'm an Egyptian citizen?

or it's common to receive this form with Iran for all the middle east.
I tried to contact them to explain my issue but with no response

My friend can you tell me what happened with you. I am from Jordan and I got the same problem.

Does this form have a name? Can it be seen online?

I do not think so, it is hard to find it online.
Then there are 7 questions asking about the education, source of funding, where would I work after graduation .....
