Didn't get to that situation during our application but I did my research, in case.

Did your spouse do her medical exam yet? If not, she will still be doing it but x-ray will probably be postponed until she gives birth. If she has done it but will give birth before coming to Canada, this will delay your application because you will then also have to add the baby to the application.

If she has done her medical exam and she will give birth after getting here to Canada, that's the most optimal situation. Just be aware that if she hasn't met the province's health insurance criteria (some require that newcomers must be in that province for a couple months before being eligible), then you will have to foot the hospital bill, unless you can get private health insurance.
 
You are not required to inform IRCC of your spouse pregnancy.......however, if you choose to inform IRCC, you only need to update IMM5532E Q. #10.

If your spouse were to deliver your baby before the PR visa and COPR is issued.....you will need to update your spousal sponsorship application with newborn.

How to update to Spousal Sponsorship application for permanent residency with a new born

If your spouse were to deliver your baby before landing as PR in Canada......you must inform IRCC of the newborn before she land as PR in Canada......as the Principal Applicant family composition has changed and IRCC must be aware of this change (IMM5406) before landing as PR.

actually, we just submit our application. based on what you replied, my spouse’s application will be delayed.
with the new addition as well as the medical will have another months to processed it.
being naturalized citizen, my citizenship can be passed to my newborn according to what i have read.

Your baby is a Canadian and doesn't need to add your baby to application as a dependent, but you need to update the form for additional family member, if your baby should be born before your wife's application is approved. The delay most likely will come from the medical exam, unless the doc allows your wife take x-ray. Also, you need to apply the citizenship certificate/passport for your baby, after he/she is born.
