However, sponsor application hasn't been started yet due to some issues matter. So during her stay with her husband, who should Pay for all medical expense while she is pregnant with her Canadian Husband ? Can she claim OHIP for that ? or is there any insurance policy will cover this expense

Pre-Assessed..
The sponsorship application needs to reach a certain stage of approval before she can qualify for OHIP so failing that, she is not eligible and she and her husband will have to pay for her health care out of pocket. If she is already pregnant, there is no insurance that I know of that will cover childbirth but there is an insurance available that can cover pregnancy complications and emergencies. You can find it here: https://arbetovinsurance.com/pregnancy-insurance-for-visitors-to-canada/

People on visitor records do not qualify for OHIP, no matter if they're married to a Canadian.
Also there is no such insurance to cover normal pregnancy/delivery, if you're already pregnant. Such insurance would have to be purchased months before getting pregnant.

So you will need to pay for all medical bills out of pocket. For a regular hospital birth this is probably around $5000 for pregnancy related costs and delivery if absolutely no complications, or $10000 for more complicated cases and if any complications involving a longer hospital stay there is no limit on the amount it could reach.

See if you can find a midwife, as their service are free even if you aren't covered by OHIP, and other costs should be very minimal. However they are in high demand so may be difficult to secure one, best to start calling them immediately after finding out you're pregnant.
