PR received but visa not stamped on one of my dependent child

Hi guys,
We received Passport request for all our family members .were submitted passports and after 4 weeks passportso were returned with visa stamped on all passports except one of my dependent child .now what to do please advise. Is it a mistake from CIC Islamabad? Please advise
