PR MEDICAL REQUEST CONCERN

Hey guys,
I have concern, If someone can please guide me.

Story - I got my PNP approved in November in winnipeg and later submitted Federal file(. And I just got request for medical and RPRF (RIGHT TO PERMANENT RESIDENT FEE) to pay.

Concerns -
1.) Do generally people get request for medical and RPRF together or at separate times?
2.) Is RPRF last stage request ? If so, it's been only 7 months so far for federal case.
3.) I been mentioned by someone processing time of federal is 18 months. Will it take total 18 months or I will get my PR in few months only ?
4.) I hope there nothing to worry about?

cic website says they try to finish all cases in 10-11 months rest of it depends on the complexity of the case also processing times for this year seems faster than last year lets hope for the best keep us posted!
