PR cards processed but stuck with Canada Post

Hi friends,

Anyone here with my situation, ...info received from CIC says that my PR cards were handed over to Canada Post on Jan 23, but have not been delivered yet ! I landed on Nov 28. Pls share if someone has come across such an unusual situation. Thanks

Thanks zardoz, few questions
1. I will have to fill these forms for all the accompanying family members also !
2. In no1, I will put my place and county of birth and same for family members !
3. In no2, I will put the address where I want them to mail the cards in Canada !
4. I. No3, I will put my Canada address where I was expecting the cards but never received, what to put in "on or about " and what should be the circumstances !
5. In no 4, what to write as I did no effort other than just calling CIC couple of times !
6. As I am outside Canada right now, should I fax it from in side canada of from here (Saudi), should these be faxed or mailed! Is it ok to send scanned copies to my fried in Canada who can fax them to NOva Scotia there on.
7. What to put in client I'd number, UCI, application number, or document number !

I'm sorry, but you have as much information as I do. I just looked up the procedure for cards that don't get delivered.
