PR Card Renewal - Port of Entry

Hello everyone! So I have to change my legal name, and thus, I'm planning to change my PR card to reflect my new name. I'm a BC resident. However, the PR card renewal form (imm5444e) asks for my port of entry and because I became a PR during the pandemic, I had a virtual landing and my port of entry on my CoPR is written as 'OSC - Operations Support Centre' and the back of my PR card says 'Ottawa" for my place of landing, despite being in Vancouver during this whole process and never having even visited Ontario before. What do I answer for my port of entry and province of entry on the application form?
