Fill it out. Get it notarized.
Attach a 13 Euro payment. You can purchase a foreign draft from a local bank for this.

They seem to do it very fast i.e. within about 3 days they will mail it out. No cover letter is required.

Actually, I already sent the request, I just need to know how long does it take them to mail it back.

I know they process and put it in the mail in 3 working days for most part. To some extent this depends on how soon your payment get cleared. So you basically need to add 3 days to the mailing, and that's the time it takes to get back to you.

I have lived in many countries. Of all of them, I got the German PCC the quickest. It simply says "No Records Found", and then adds that since it was a computer search, the certificate does not have a signature. I am sure you will get it soon, esp. if the mailing system of your country is quick. Best of luck ;D
