My husband requested it and we just got it in the mail....it doesn't say Option C on it but its got all tax infpr for previous taxation year.....and it says RC143-1-E on the bottom....is that it!? That's what they call option C!?!

I think so. I found this (from another website) that seems to confirm that it is:
"I am looking at an Option C printout right now and it says RC143 E(04) in the top right hand corner. The heading is Income Tax Return Information - Regular. No where on it does it say Option C."
