Hi, to claim points for adaptability you have to have both birth certificates to proof that you have at least on parent in common and you are brother/sister, in some countries yo can ask for a family booklet also that is valid. Besides the certificate you have to send a proof that your brotther or your wife´s brother lives in Canada like a lease contract a mortage contract and taxes slips as well as the copy of his/her passport or resident card.

About medical exams they will ask you later on (Canada´s Government) not at initial phase.

for point
2.also my wifes name was changed after mariage does any one know what document needs to be produced as leagl document showing name change -- Marriage Certificate

If birth certificates are not available can you order new ones? If not are there other documents that would be acceptable such as school leaving certificates? Check the FSW application document checklist and guide to see what can be substituted for a birth certificate and try to find one of those things for both.
