POF: HDFC Bank Jaipur India, anyone?

Issue: One of the HDFC bank branches in Jaipur is refusing to provide the custom letter that will include all the information CIC require. They have provided a letter that is just a balance certificate (not even on the letterhead)

Anyone got the POF letter from the HDFC bank in Jaipur, India?
Any information will be very helpful, Thanks in advance

Hi...did they gave CAD equivalency on the balance certificate? Could u pls help with the format you used.
