I have paid using credit card online, and not filled that form. As far as I know, if you are not paid online using credit card and have an international credit card to pay which will be deducted in future when CIO open your application then you need to fill that form.

There is a declaration portion at the end of IMM 5669, background form. We are told to leave it and not sign it yet.

Is that correct, can we leave it unsigned?

Yes, it should be unsigned. Just sign on the immediate last portion of this declaration.

Do you know imm 5690 is recorded on the site as updated Jan 2013 but if you print d form you will see 03-2014

Yes, u r right. As long we are downloading from their site and we need not to write the date anywhere, we are safe.
