Use your judgement on filling in dates and activities etc. N/A may not be appropriate.

if I did bachelor's from 1998-2001 and then master's 2004-2006, i wud state: 2002-2003 - preparing for higher studies/entrance exams etc. Not just N/A.

no proble, ieven if you have not test result you can apply, test proof is required at 2nd stage.
if result expires then u need to appear 2nd time in test

my SSC - passed on 1999

HSC - 2001
BBA - Four years starts from 2002 but it tooks me till 2007 - 06

I have the same concern, can I write my job description in a separate sheet of paper or just a brief description that fits on the form.

One more thing, why should they ask that IELTS should not be more than 1year old, isn't IELTS valid for 2years? I took IELTS last may 2009, does it mean I have to take it again?
