Please Help - Pakistani Student Visa

I have received acceptance for MSc program , I am in the process of applying for Visa to CHC Islamabad. I have two questions regarding documents that need to be submitted with the visa application.

1. CNIC - Do i just need to submit the notarized copy of my CNIC in Urdu or do I need to submit the English translation as well?

2. Family Information form - It says write the names both in English as well as your native language(in my case urdu), so should I write both English and Urdu names on the same page or should I print two copies, one with English names and the other with Urdu names?


If anyone could help with these queries, it would be really helpful.

1: You do need to submit English translation as well.

2: Some people do write their name in Urdu on the same page (after printing) but it's not necessary at all ,you can just print you name in English and that's it.
