You’ll need to be a PR or citizen first. Then you’ll need to pass the licensing exam. You’ll need to do a Canadian residency for a minimum of 3 years. The big issues is that once Canadian students have selected their residencies there aren’t many spots left so international medical graduates (including Canadians returning after attending medical school abroad) all have to compete for the leftover spots. There are many people applying so many don’t secure a residency even after a few tries. The residencies leftover are also usually in less desirable locations. Any IMG coming to Canada must know there is a risk they will never practice in Canada. If you search the internet you can read personal stories. If you want a place where you are guaranteed to practice and speak English would look at the UK, US and maybe Australia or NZ.
