Finally, I am ready to send the package in 2 days. I thought before I send it I tell everyone here what I am including just to get a last minute double-check.

1. Online payment receipt
2. Document checklist
3. Application
4. Copy of landing paper + Copy of PR Card
5. Copy of Driver's License + Copy of Health Card ( for 2 pieces of ID)
6. Copy of all pages of Passports (I have 2 passports including an expired one, so made copies of all pages for both)
7. Original of CBSA report (that I ordered and received last month)
8. Original notarized translated report of non-English exit/entry stamps (from expired passport)
9. Copies of last 4 year's NOA & a current pay-stub from employer
10. Copies of University Degree & Certificate (from States) to satisfy English language proficiency requirement
11. 4 copies of photos per specification (with name written in the back)

Am I missing something? Do I need to include a cover-page?

Also note that the application form was updated about a week ago to CIT 0002 (05-2014) E .

Is there any particular reason you are submitting 4 photos when the check lists asks for 2? Also, I'm not sure about the value of including your CBSA report as I'm sure they'll just request it if you've given them authority. I have mine but wasn't going to submit it, but I'm happy to be convinced otherwise!

I would skip these :
6- copies of aaaaallllll pages of passport. It's not required at this stage. Just bio pages for now.
7- They updated the application form (hope you are aware of that) specifically to tell you DO NOT ORDER YOUR TRAVEL RECORDS FROM CBSA. so if you include it you're telling them I don't give a s** at what you say.
8- not required at this stage.
10- you need to include transcript if using a degree as proof of language proficiency.
11- 2 photos only !
You only need a cover page if you need to add some details, explain something they may not understand , otherwise, no need.

I had completed the application two weeks ago and was gathering rest of the documents. Thank you for letting me know.

OK, I will not include the CBSA report with the package, however I thought I was required to submit all pages of passport if I had traveled and the passport has exit/entry stamps.

IS THERE ANYTHING NEW IN THE NEW FORM?
 
Are the NOAs really required?

Read question 6.E about work history in the form. It asks for work or school records.

You do not need all pages of passport when you apply. If you receive RQ yes, they will ask for all copies of all passports including blank pages with no stamps. Please read the guide on the website.
The only difference between the form(Feb version ) and the latest one (May version) is that they ask Not to order travel history report from CBSA. The rest is exactly the same.

NOA is Ok but if you can add T4's as well that's even better and maybe employment letters

Actually, I have only last year's T4 as I worked as salary employee only last year. Before then I worked as a commissioned employee however I didn't get T4. Before that I was self-employed. So, except the NOAs, it would be difficult to organize T4s for the previous years. I can include a current pay-stub if that helps.
 
I am hearing from some people that including NOAs is not necessary. Is this correct? Does it hurt or help if I include NOAs for last 4 years? (I don't have any T4)

Do u have anything better to include for question 6.E? Also are you sure you read question 6.E and understood what it's asking for? If NOA is not necessary and you don't have T4's what are you providing as WORK RECORD as specified in question 6.E ??

Are your NOAs in line with the earned income despite the (odd) lack of T4s? What does your employer state when you ask for the T4s?
