A Filipino minor traveling from the Philippines to a foreign country accompanied by a person other than his or her parents.


Who does not need a DSWD Travel Clearance to leave the Philippines?

A Filipino minor traveling with either parent, with his or her solo parent or legal guardian;
A Filipino minor whose parents are in the Foreign Service
A Filipino minor who holds a temporary or permanent resident visa of another country and is rejoining the parents in that country
A child holding a foreign passport

Hi there sis...with regards to DSWD don't worry ... all minor travelling toa foreign country with either parent or with his/her solo parent or legal guardian either legitimate or illegitimate does not need travel clearance....yung form IMM5604 lang talaga from the embassy and problem ko ...my husband in canada called the embassy there and he was advised to send a copy of full custody from the court ....my god..i dont have enough time na for that really ... we are waiting for ppr now and if i will file it to court it would take months according to my sister in law and shes a lawyer, it's really a big headache on our part now... so the only thing i can do now is to take the advise of one of our friend here in the forum to make a letter and notarized it or else i should track down my x and let him sign the IMM5604 .... God please help us....

Hi sis.. From what I've read before sa mga past post na may situation like yours all they did is make a written explanation na wala na talaga kayo communication nun father & hindi mo na alam kung san sya mahahanap.. And better if pa notarized mo or hingi ka din ng supporting letters from your family & friends na ganoon nga ang nagyari sa inyo... Pero sis try to ask others din kasi di naman ako expert sa ganyan maybe they can give you a better advise... Don't worry sis you'll get through... Maayos din yan.. ☺☺☺

I had my medical in St. Lukes Extension Clinic sa may Jorge Bocobo sa Manila. I paid 4K. The staff and even the doctors are nice.Same day din nakuha ko copy ko for my medical, na pinadala ko sa husband ko along with other supporting documents.
Sa St. Lukes as early as 7 am they are accomodating patient for Canadian Visa applicant. The receptionist for the Canadian Visa applicant informed me before I left na if within 7 working days I don't receive any call from them that means ok naman ang medical and no further test needed.

Eto website nila if in case may question ka www.slec.ph.no problem ka sa dental and eyes kasi po kung ma find out n need kang mag pa eyeglasses will it affect the medical?

May physical examination na ginagawa. Iba yung nagtitingin ng sa eyes at iba naman yung nagtitingin ng sa teeth. Initially naman tatanungin if you are wearing eye glasses or not. Sinabi ko oo since nagsasalamin ako.Tinignan lang yung eye glasses ko tapos pinabasa lang ako without wearing my eye glasses. Wala naman akong naging problem.

At that time when I had my medical pinabalik ako ng 2 pm to get the copy of the proof na nagpamedical na ako. Same paper na sinama ko dun sa application ko na pinadala ko sa husband ko para ma-forward nya yung papers naming sa Mississuaga. Then I was informed na kung within 7 working days wala akong mareceive na tawag from them that means ok na yung blood test.
