Suppose someone has studied a one year program and then got work permit for 12 months. What is he supposed to do after 12 months? Can he extend his work permit? Or he should come back to his home country and will never be able to go back to Canada?

You can’t extend a PGWP. You need a LMIA and then apply for a closed work permit.

then you will lose your legal status and have to leave canada. unless you get job offer and apply for work permit either lMIA or LMIA exempt. you need at at least 1 year of work experience in Canada to be eligible for PR through CEC. hence 1 year pgwp is not the safest route.
