personal identification for child under 14

Please guide what personal identification for child under 14 required, one with photo. health card for under 14 has no photo. school report no photo. copy of passport and PR Card are being submitted as reqired in other columns but can it be considered personal identification.

1. Passport (country of origin or birth)
2. PR Card (Canada)
3. National ID Card (of the country of origin)
4. School Report Card, or School ID Card if available
5. Health Card

Even without photo, no harm in sending a copy of them. Even if 1, 2 or 3 are good enough
