An elderly lady with a still valid PR card is going to return to Canada soon. Will she have to prove that she met her residency obligations to the border officer? Or all entry-exit info is already there in computers? She speaks no English or French.

It's always possible she may be questioned about whether she meets RO. It's also possible to be reported at the border for failing to meet RO.

They have translators.

They are supposed to have translators, but in reality they often don't (personal experience). What proof she has to possess? Passport stamps, what else?
And please clarify your second sentence: reported where?
 
Does she meet the RO? Then it shouldn't be a problem even if there may be some confusion. If you can prove you've met your RO you will be able to stay in Canada.
